Use Special Form if:
You have had Primary class instruction with a teacher of Christian Science.
Your teacher will not be signing your form for any one of the reasons listed in the Manual of The Mother Church.

Download the PDF form.
Either:
Complete the form electronically then print.
Or print the form and complete it by hand.
Sign your printed form by hand. (Electronic signatures not accepted.)
Give your form to three members of The Mother Church who know you well to check over your application, and sign it as approvers.
After all three have signed, mail your form to the Clerk of The Mother Church or scan and email it to clerk@christianscience.com.
That’s it! Once we’ve reviewed your application, you’ll be considered for the next admission of members, either in May/June or November.
