You have not had Primary class instruction with a teacher of Christian Science.

Or print the form and complete it by hand.
Sign your printed form by hand. (Electronic signatures not accepted.)
If you are considered a minor in your country, have a parent or guardian sign the permission section of the form.
Give your form to a member of The Mother Church who knows you well to check over your application—they will sign it as an approver.
If the approver is a Christian Science teacher (CSB), they will sign and mail or scan and email your application to the Clerk of The Mother Church.
If the approver is not a Christian Science teacher, they will sign, then forward your application to a Christian Science teacher (CSB) for an additional signature. The teacher will act as a countersigner and mail or scan and email your application to the Clerk of The Mother Church.
That’s it! Once we’ve reviewed your application, you’ll be considered for the next admission of members, either in May/June or November.
