What a great journey you are beginning! Let’s get you started on your way. There are three forms provided in the Manual of The Mother Church. You only need to complete one of them.

Form 1

  • If you have had Primary class instruction with a teacher of Christian Science

  • Your teacher will sign your form

Use Form 1

Form 2

  • If you have not had Primary class instruction

Use Form 2

Special Form

  • If you have had Primary class instruction with a teacher of Christian Science

  • Your teacher will not be signing your form for any one of the reasons listed in the Manual
